The parking brake cable on my 1992 is dangling with about 3 inches of slack, not tight, beneath the driver side front passenger door.

 

In a nutshell, what do I to fix this?

New cables.  If it's that far stretched, no amount of adjustment is going to fix it.  It could even have broken somewhere.

Okay figured it out.   The mechanism on the caliper itself wasn't pulling out the slack.  I don't use the e-brake that much, so that part of the mechanism on the driver side rear caliper had sort of seized.  About 50 cycles and blows with the hammer later, it seems to be working better.

Using the park brake keeps the rear calipers adjusted.  You don't use the park brake, you have caliper problems.
